Below is a list of everything you may need to BRING to your tax preparation visit. carefully review the list and determine which apply to you and your family:
Valid (government-issued) photo ID for yourself and your spouse
Social Security or Individual Tax Identification Number (ITIN) card or letter for you, your spouse and all dependents
Birth dates for you, your spouse and all dependents
Bank routing and account numbers for checking and savings accounts.
Forms W-2 from every place you worked in 2026 and Forms 1099, if applicable
Interest and dividend statements from banks (Form 1099-INT or 1099-DIV) and any stock sales tax forms.
All 1099 forms including Social Security Statement (SSA-1099), Pensions (1099-R) and Unemployment Statement (1099-G) if applicable.
If you have a pension, bring the month and year that you received the first payment.
If self-employed, bring 1099-NEC, 1099-K (Uber, Lyft, DoorDash, etc.) , and all expenses with this completed Self-Employment Worksheet
Tuition expenses paid for you or your child to attend a college or university (Form 1098-T)
Student loan interest statement (Form 1098-E)
Health insurance information (Form 1095 A) if purchased through the marketplace (Healthcare.gov statement)
Child care expenses including (total amount paid, provider’s name, address, and tax ID or social security number)
Other relevant information about income and expenses, i.e., home interest, property taxes & medical expenses for those who will be able to itemize
Records for all charitable donations even if not itemizing your returns. Arizona taxpayers can claim charitable donations, including those to schools, and may qualify for state tax credits.
If filing jointly, you and your spouse must be present to sign the returns
Download, complete, print and bring with you this INTAKE PACKET AND SUPPLEMENT INTAKE FORM (if unable to print, please arrive 15 min. early to complete these forms)
A copy of last year’s tax return (if available)
Letters received from the IRS or State Department of Revenue
